There is an open Per Diem Nurse Practitioner (NP) Home Health Flexible Schedule job in Vernon, CT. The Per Diem Nurse Practitioner (NP) will conduct home health risk assessments within Tolland County, CT. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Vernon, CTand the surrounding Tolland County, CT. Tolland County, CT includes the cities of Vernon, Ellington, Stafford and Mansfield. Candidates will need to be willing to go to any of these individual locations for their shift.

Responsibilities:

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
• Provide education on medical conditions and medications

Work Hours:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Preferred Experience:

• 1 year of current NP experience is preferred

Preferred Qualifications:

• Nurse Practitioner License in CT
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC

Education Requirements:

• MSN degree from an accredited school of nursing
